孕妇用药分级:D 级,在对照的人体研究试验中显示该药物对胚胎有不良影响,若此药能带来之效益远超过其它药物的使用,因此即使在其危险性的存在下,仍可接受此药物用于怀孕妇女上。

药理作用:Cyclophosphamide是由oxazaphosphamide基团衍生而出的细胞抑制剂,结构上和芥子气相关。体外不活化,须经由肝脏内的微粒体酵素活化成4-hydroxycyclophosphamide,和aldophosphamide为立体异构物。其细胞毒性作用为烃基性代谢物和DNA起交互作用而来,使DNA双股产生错误的交叉连结及蛋白质结构改变。细胞循环中,经由G2期的通道是退化的,细胞毒性作用对于细胞週期阶段不具特异性,但对细胞週期则有。和其他烃基化物一样,尤其是构造相关的细胞抑制剂,如Ifosfamide,交叉抗药性不能被排除。
适应症:Hodgkin病,非Hodgkin淋巴瘤(Non-Hodgkin’s lymphomas)滤泡淋巴瘤(Follicular lymphomas)网状细胞肉瘤(reticulum cell carcoma)淋巴母细胞的淋巴肉瘤,Burkitt’s淋巴瘤(首选用药),多发性骨髓瘤白血病(Leukemias)。慢性淋巴性白血病(Chronic lmphocytic leukemia)慢性颗粒性白血病(Chronic leukemia)急性骨髓性和单核细胞白血病(Acute myelogenous and monocytic leukemia)。急性淋巴母细胞白血病(acute lymphoblastic leukemia)蕈状肉芽肿(Mycosis fungoides)神经母细胞瘤(Neuroblastoma)卵巢腺癌(Adenocarcinoma of Ovary)网膜母细胞瘤(Retinoblastoma)(首选用药),乳或肺癌(首选用药)。
用法用量:口服,成人─1天1~5mg/kg;孩童─成人:2~8mg/kg或60~250mg/㎡,共6天或6天以上。维持量:2~5mg/kg,或50~150mg/㎡每週2次。
副作用:接受Endoxan治疗的病人,副作用的产生依剂量而定,在大多数的案例中市可逆的。最常见的副作用是噁心、呕吐。
禁忌:1.对cylcophosphamide过敏者。2.骨髓功能严重受损者(尤其是曾以cytotoxic agent治疗或接受放射性治疗的病人)3.膀胱炎。4.尿道阻塞。5.感染症(active infection)。6.怀孕及授乳期。
注意事项:有下列情况之病人应用Endoxan时应注意:1.白血球、血小板减少症。2.骨髓之肿瘤细胞浸润。3.以前用X光治疗过。4.以前用其他的Cytatoxic agent治疗过。5.肝功能损害。因为Endoxan有抑制免疫机转之作用,病人如引起细菌、霉菌或病毒之感染应停药或更改剂量,尤其是与类固醇药物合併治疗,有时会使病人死亡。在这些情况下,水痘-带状泡疹之感染尤其危险。
保存方式:药品应置于摄氏 15 ~ 25 度乾燥处所;如发生变质或过期,不可再食用。

This medication is classified as an “alkylating agent.”USESCyclophosphamide is used for treating certain types of the following cancers: lymphoma, multiple myeloma, leukemia, mycosis fungoides, neuroblastoma, ovarian cancer, eye cancer, and breast cancer. It is usually used in combination with other medicines. It may also be used to treat certain kidney problems (nephrotic syndrome) in children or for other conditions as determined by your doctor.SIDE EFFECTSAppetite loss; absence of menstrual periods; color change in skin; diarrhea; general unwell feeling; hair loss; nausea; skin rash; stomach discomfort or pain; texture change in nails; vomiting; weakness.Seek medical attention right away if any of these SEVERE side effects occur:Severe allergic reactions (rash; hives; itching; difficulty breathing; tightness in the chest; swelling of the mouth, face, lips, or tongue); blood in urine; bloody or black tarry stools; chest pain; chills; fever; hallucinations; increased or decreased urination; infection; lower back or abdominal pain; pain, swelling, or redness at the injection site; painful urination; persistent cough; pneumonia; red, swollen, or blistered skin; seizures; severe stomach pain; shortness of breath; sore throat; sores in the mouth; swelling of the hands or feet; unusual bruising or bleeding; unusual or severe tiredness or weakness; yellowing of the skin or eyes.This is not a complete list of side effects and others may occur.
